Dr. Tenma finally has the job he's always wanted. Head neurosurgeon at a huge hospital in Germany. He is the most skilled surgeon the hospital has ever seen and has saved the lives of patients who would have died at the hands of any other. But work isn't always as pleasing as he would like. Instead of treating patients in the order they arive, or most critical first, the hospital has him treating people of the most importance before anyone else. Fed up with this rule, Tenma defies the Hospital head one night when a young boy, shot in the head; and his sister, in a state of shock; come in before the Mayor. Their parents had been murdered and the two siblings were the only survivors. After Tenma saved his life the boy, named Johan, mysteriously disappears along with his sister. All the Hospital heads are dead, with Tenma being an obvious suspect.
Now, several years after saving Johan's life, murders are surfacing with the only clues being left by those who broke into the victims houses. The sole survivor of which gets hit by a car and lands in Dr. Tenma's care. He claim's that a monster is coming and that he was hired by a man named Johan. Tenma is determined to find out how Johan is connected to the murders and leaves his job behind for the chase. Who is this mysterous Johan, and why is he killing so many people?

He is the most skilled surgeon on the hospital's team and has performed countless successful operations that had they been performed by any other would have ended in disaster. He's an Asian man who has come to Germany in hopes to further his medical studies and work in the German hospital. But it was his skill that set him upon the path of chasing down a serial killer.

Anna, aka Nina Fortner, was one of the two survivors when her foster parents were killed. The only other survivor was her brother Johan, who was shot in the head. Eleven years after the incident, Anna has no memory of her brother. When she meets Tenma, her memory comes flooding back.

Shot in the head after his parents were killed, Johan is treated by Tenma. He mysteriously disappears from the hospital one night, leaving behind several important hospital figures dead, including the hospital director. Now he's nowhere to be found, and over the past eleven years there have been several murders of foster parents. Tenma believes Johan is somehow connected to these murders, but does Johan have other plans in mind?

The Inspector
Inspector Lunge is a very odd man indeed. He moves his fingers as he talks as if he were typing on a keyboard. He was selected to deal with the murder cases of the foster parents. Lunge has high beliefs that Tenma is the murderer in all the cases, and refuses to believe otherwise.

The Hospital Head
Heinemann is the hospital director. He is in charge of who opperates on what patient. Heinemann thinks that patients should be opperated on in the order of their importance, rather than the order they arive or by judge of how critical a condition they're in. He knows that Tenma is the best surgeon he's got, so he takes Tenma off alot of his patients to make him opperate on people of more importance, like the mayor.

The Fiancé
Eve is the hospital director's daughter. She and Tenma had been engaged but when things began to go awry, the wedding got called off and she left him for another highly titled surgeon. However, despite the fact that Eve was the one who left Tenma, she still wants him back.

The Boy
Dieter is a young boy who Tenma rescues in his efforts to gain knowledge about Johan. He comes across a man who was fostering Dieter, an orphan, and who had known Johan when he was still a child. Tenma caught the man abusing Dieter, and so took Dieter with him on his journey.
